Health and Wellness Benefits

HEALTH AND WELLNESS

Medical Insurance

  • Employees who work at least 30 hours per week - premium is paid partially by the college. For rate information click here.
  • Two medical plans available: 1. Traditional PPO 2. High Deductible Health Plan (HDHP) with an optional Health Savings Account/HSA (see below for additional information on HSAs).
  • Both plans include in-network and/or out-of-network benefits, major medical, and prescription card. (The Traditional PPO has a $100 vision benefit; the HDHP does not.)

Dental Insurance

  • Employees who work at least 30 hours per week - premium is paid 75% by the college, 25% by the employee. View rate information.

CSM utilizes the Guardian DentalGuard Preferred PPO. To locate "in-network" dental providers, visit https://www.guardiananytime.com , click on Find a Provider, click on "PPO" under the Dental Plan drop-down menu, and click on "DentalGuard Preferred" under the Dental Network drop-down menu. The plan provides both in- and out-of-network benefits. Using in-network providers results in savings to you. If you do not wish to use an in-network provider, you will receive the lesser out-of-network benefit.

Total yearly maximum benefit is $1,800 per covered family member. Orthodontia benefit is provided for children only to age 19: $1,500 maximum lifetime benefit (per child).

Vision Insurance

Health Savings Account

If you elect to enroll in the Qualified High Deductible Health Plan (HDHP) for the new plan year beginning 7/1/15, you can open a Health Savings Account through Bank of America. If you have single coverage, you can elect up to $3,350 for the calendar year and if you have family coverage, you can elect up to $6,750. Enrollees age 55-64 can contribute an additional $1,000 catch-up amount. If you need an enrollment form, please contact HRD.

Flexible Spending Accounts

Healthcare Spending Accounts:  Reduce your taxable income—the amount you elect is deducted from your paycheck on a pre-tax basis.  Submit claims to PayFlex for eligible out-of-pocket health care expenses that are not covered by insurance (office visit & drug copays, medical/dental deductibles, coinsurance, orthodontia, laser eye surgery, hearing aids, etc.)  PayFlex issues reimbursements directly to you.  The maximum deduction for healthcare spending accounts is $2,550 for the 7/1/15 – 6/30/16 plan year.  You may participate in a healthcare flexible spending account even if you are not in the CSM health plan. 

Dependent Care Spending Accounts:   These accounts allow you to save money on dependent care expenses by having an amount deducted from your paycheck on a pre-tax basis.  Submit claims to PayFlex and they reimburse you directly.  The maximum deduction is $5,000 for the 7/1/15 – 6/30/16 plan year.

Domestic Partner Coverage

Domestic partners and their dependents are eligible for the same CSM medical and dental benefits as those provided to married spouses and their dependents. The College of Southern Maryland defines domestic partners as two (2) individuals, of the same or opposite sex, who live together in a long-term relationship of indefinite duration. There must be an exclusive mutual commitment similar to that of marriage, in which the partners agree to be financially responsible for each other's welfare and share financial obligations, as verified by affidavit.

The value of insurance benefits provided to the domestic partner and his/her dependents is considered taxable income to the employee by the Internal Revenue Service and is subject to social security and federal and state income tax withholding.

Life Insurance

  • Eligible full-time employees receive basic life insurance in the amount of 2 times salary, to a maximum of $100,000, at no cost to the employee.
  • Supplemental life insurance in increments of $25,000, to a maximum of 5 x annual earnings or $500,000, may be purchased (rates are age-based). Medical underwriting is required for amounts over $250,000. Spouse coverage is available up to 50% of employee's coverage amount, to a maximum of $75,000. Child Life is also available in the amounts of $5,000 or $10,000 (covers one child or multiple children, inclusive). Paid 100% by the employee.

Part-time employees are not eligible for life insurance.

Long Term Disability

  • Eligible full-time employees receive basic long-term disability insurance, which would pay a benefit of 60% of monthly base pay ($6,000 monthly maximum) beginning on the 90th day of a disability. There is no cost to the employee.

Part-time employees are not eligible for LTD insurance.

Short Term Disability (SICK LEAVE POOL)

Eligible full-time employees may join the Sick Leave Pool to ensure a measure of income protection during the LTD elimination period, (the 21st work day to the 90th day of disability). Employees may enroll within the first 30 days of full-time employment or during open season by donating 3 non-refundable days of sick leave to the pool. Sick leave pool days are paid at 75% of salary.

Part-time employees are not eligible for the Sick Leave Pool.

Employee Assistance Program (EAP)

LifeWorks provides free, confidential help with personal and work-related issues 24 hours a day, 7 days a week. Employee Wellness Center Activities

The Wellness Center helps individuals attain an optimal level of health and well-being. The center sponsors contests, offers positive lifestyle courses, lunch-time learning seminars (Health Breaks), health screenings, and informative pamphlets about a variety of health-related topics.

Time-Off for Fitness Program

Employees who participate in wellness activities on CSM campuses are able to use up to one hour per week (additional 30 minutes 2 days per week), Monday through Friday, for fitness activities, with their supervisor's approval. Non-exempt employees on time sheets should record this time in column 8 (Other) on their time sheet. An employee will be required to come in earlier, stay later, or shorten his/her lunch break to compensate for any time used in excess of the additional half-hour.

Fitness Exercise Lab

Fitness Exercise LabFitness centers are located on the La Plata, Leonardtown, and Prince Frederick campuses and are open to all employees free of charge. The centers house various strength training equipment, free weights, exercise bikes, treadmills and elliptical trainers. The LaPlata campus offers an indoor pool which is also free to employees. Individualized fitness equipment orientations are provided free of charge by appointment. Physical fitness assessments are offered by certified Fitness Trainers for a minimal fee. Employees may register for group fitness classes. No tuition is required for the classes, but employees are responsible for paying any course-related fees.

All employees may use the Fitness Exercise Labs and pools by swiping their One Card to gain entry. Employees will have to complete health forms confirming that they are physically able to exercise and have no restrictions from their doctor(s). WFA personnel will explain the use of the equipment if requested.

Dependent spouses and children of benefits-eligible employees may purchase individual memberships and pay fees only (approximately $414.00/year as of 2015). Fees are subject to change.
