If you checked yes to one or more questions on the Health Status Questionnaire on the original five-page registration form, you will be required to have your doctor complete this form prior to the first day of camp. Our Registrar's Office should send you a letter, if this is required soon after you register.

*Parent letters will be mailed early June, for SYC, SSC, and AAY Camps.

If you need to drop a class or add another class; complete the Add/Drop form.
