It Takes a Community to Care

The CSM Collaboration, Assessment, Response, and Evaluation (CARE) Team is committed to fostering an inclusive, safe, and connected campus. The work of caring for students and our community falls to all of us, however. The CARE Team is here to guide you on when and how to submit a report should you have a concern about a student or other community member. Thank you for being part of a community that looks out for one another.

About the CARE Team

We Strive to:

  • Build a campus culture of care, support, and safety
  • Proactively identify and address behavioral concerns
  • Offer timely support and effective interventions
  • Use evidence-based practices and respectful communication

What to Report

Submit a CARE referral if you observe:

  • Significant changes in mood or behavior
  • Threats to self or others
  • Signs of distress, isolation, or withdrawal
  • Safety concerns or alarming conduct
  • Any behavior that may impact well-being or community safety

The CARE Team approaches every situation with compassion and professionalism. Together, we work toward solutions that respect the individual and strengthen the CSM community.

CARE Referral Awareness

Submit a CARE Referral

Concerned About Someone at CSM? Make a CARE referral to support the well-being and safety of our college community. Whether you're witnessing concerning behavior or simply worried about a fellow Hawk, submitting a referral helps CSM respond with care.


If this is an emergency, call 911.

Do not use the CARE referral form if an immediate response is needed.

What Happens After You Submit?

  • Acknowledgment: You'll get confirmation your referral was received.
  • Assessment: The CARE Team reviews reports and evaluates concerns on a regular, ongoing basis. 
  • Response Planning: A tailored support plan is created based on the individual's and college community's best interests.
  • Confidentiality: Updates may not always be shared to protect privacy.
