How to Apply for Services

Please make sure you have completed all steps in order to receive the services you need. All forms can be downloaded and printed using the links below.

  1. Make an appointment with Disability Support Services (DSS) for a new student intake meeting with the ADA Coordinator or Disability Support Services Coordinator.
  2.  Complete “Request for Accommodations” form
  3.  Read and sign “Rights and Responsibilities of Students with Disabilities” form
  4.  Read and sign “Rights and Responsibilities of the College of Southern Maryland” form
  5.  Read and sign  “Procedure for Requesting Services” form
  6.  Provide appropriate documentation of your disability. If you do not have documentation you can still schedule a meeting with a Disability Support Staff to determine appropriate documentation as well as temporary accommodations if needed.
  7.  You should be able discuss your disability and the impact of your disability in an academic setting with the DSS staff.

If you do not have appropriate documentation, we can help you find a medical professional who can evaluate you. The DSS has a list of psychologists in the metro area who provide assessments for learning disabilities.

  3. Contact DSS to review and pick up your accommodations plan.
  4. Have all faculty sign the master accommodations plan and give them a copy.
  5. Return master accommodations plan to DSS with all faculty signatures and your signature.

  6. Provide appropriate documentation of your disability from your medical professional. If your documentation is determined insufficient, you can meet with the ADA coordinator to determine appropriate and reasonable temporary accommodations
  7. Discuss the impact of your disability on the learning process and discuss appropriate and reasonable accommodations with the DSS staff

It is important to remember that all Summer Camp and Kids' College students are treated according to ADA regulations. Because it is a postsecondary institution, no College of Southern Maryland programs are a part of IDEA.

  3. Provide appropriate documentation of your child's disability from your medical professional. If your documentation is determined insufficient, you can meet with the ADA coordinator to determine appropriate and reasonable temporary accommodations
  4. Discuss the impact of your child's disability on the learning process and discuss appropriate and reasonable accommodations with the DSS staff

Disability Procedures
CSM is committed to making individuals with disabilities full participants in its programs, services, and activities in compliance with the American with Disabilities Act of 1990, Section 504 of the Vocational Rehabilitation Act of 1973, and all other pertinent laws. This includes individuals with documented disabilities interested in participating in Kids' and Teen College. Individuals with disabilities who require special accommodations in order to participate in the college’s instructional programs should notify the ADA Coordinator on the nearest campus at least one month before the class begins. Requests made after this deadline will be considered on an individual basis and addressed whenever possible.

It is very important to have your documentation correct and up to date to receive services. Temporary accommodation can be provided while you are obtaining your documentation. A list of psychologists in the metro area can be provided by DSS for those who need psychoeducational assessment. Additionally, you can find out what documentation is sufficient for your disability by downloading the guidelines below:

Additional Forms

In addition to the general request forms, there may be other forms you will need to fill out to receive certain services. You can download these using the links below. However, we do not recommend that you download them unless you have been advised to do so by DSS staff. Save a tree; don't print it unless you are sure you'll need it!
