To request reasonable accommodations and support services through the Office of Disability Support Services (DSS), please complete these five simple steps:
The application form will need to be submitted every semester in which you require accommodations. Accommodations may change from semester to semester and will not be implemented retroactively.
Access the forms. If you need assistance completing the form or a paper copy, please contact Disability Support Services.
All accommodations are determined on a case-by-case basis, according to the needs of the individual and the individual’s specific program of study for a particular semester.
Documentation should be current and will be evaluated on a case-by-case basis. Documentation may be sent by email or mailed or dropped off in person.
After receiving your paperwork, the DSS staff will schedule an initial appointment to discuss barriers to access, accommodations, and services that you may need.
After you receive the email notice confirming your approved accommodation plan. Present your instructors with your Approved Accommodations Plan to coordinate accommodations. It's good practice to also have a confidential discussion with your instructor about your accommodations shortly after giving them the letter.
You aren't required to disclose medical information to your instructors.
Pay attention to what's working, what's not working, and what kind of barriers you're still experiencing throughout the semester. Your accommodations aren't set in stone.
We want to hear about what's been working, what barriers still exist, and what you think could help. Please notify the office of Disability Support Services right away if you encounter any serious barriers to access at CSM.
If you do not have appropriate documentation, we can help you find a medical professional who can evaluate you. The DSS has a list of psychologists in the metro area who provide assessments for learning disabilities.

It is very important to have your documentation correct and up to date to receive services. Temporary accommodation can be provided while you are obtaining your documentation. A list of psychologists in the metro area can be provided by DSS for those who need psychoeducational assessment. Additionally, you can find out what documentation is sufficient for your disability by downloading the guidelines below:
